مقدمه

Cholera is an important public health problem worldwide. Although most cholera infections are not detected, large cholera outbreaks, such as those seen in Haiti, Viet Nam and Zimbabwe in recent years, can occur. Industrialized countries have seen practically no cholera cases for over a century because of their good water and sewage treatment infrastructure. However, the causative agents (Vibrio cholerae O1 and O139) continue to thrive wherever crowded housing conditions exist and water and sanitation facilities are suboptimal (Mohammad Ali Anna Lena Lopez; Young Ae You; Young Eun Kim; Binod Sah; Brian Maskery; John Clemens (1981)).

In 2009, the number of cases of cholera reported to WHO increased by 16% when compared with 2008. A total of 221 226 cases, including 4946 deaths, were reported from 45 countries; the case-fatality rate (CFR) was 2.24% . Global trends during the past 20 years have shifted from a high incidence in the Americas during the early 1990s to a high incidence in Africa, with few cases reported from Asia (Fig. 1) (WORLD HEALTH ORGANIZATION Geneva (2009)).

The World Health Organization (WHO) maintains a public database of cholera cases and provides outbreak updates and an annual summary of aggregate national data in the Weekly Epidemiological Record. Owing to its rapid spread, cholera was one of three diseases requiring notification to the WHO in the 1969 International Health Regulations (IHR), and today, after the 2005 revision of the IHR, cholera outbreaks still require notification. Despite this, cholera notification remains incomplete because of inadequate laboratory and epidemiological surveillance systems and economic, social and political disincentives to case reporting. WHO estimates that the officially reported cases represent only 5-10% of the actual number occurring annually worldwide. Of the estimated 3 to 5 million cases that occur globally every year, about 100 000 to 120 000 die (Mohammad Ali Anna Lena Lopez; Young Ae You; Young Eun Kim; Binod Sah; Brian Maskery; John Clemens (1981)).

Under the International Health Regulations (2005) official notification of all cases of cholera is no longer mandatory, however public health events involving cholera must always be assessed against the criteria provided in the Regulations to determine the need for official notification. The move away from mandatory notification linked to automatic publication should contribute to improved surveillance as well as improved information-sharing to prevent and contain cholera epidemics. Local capacity for improved diagnosis, data collection, compilation and analysis needs to be strengthened so that vulnerable populations living in high-risk areas may be identified and offered comprehensive control activities. Cholera surveillance should be part of integrated surveillance systems that include feedback to the local level and information-sharing at the global level. Using WHO’s standard case definition will allow for a more precise estimation of the cholera burden and thus allow for more sustained support (WORLD HEALTH ORGANIZATION Geneva (2009)).

Experience shows that quarantine and embargoes on the movement of people and goods are unnecessary and ineffective in controlling the spread of cholera. The isolated cases of cholera related to imported food have been associated with food in the possession of individual travellers. Consequently, import restrictions on food produced under good manufacturing practices, based solely on the fact that cholera is epidemic or endemic in a country, are not justified (WORLD HEALTH ORGANIZATION Geneva (2009)).

V. cholerae O139 emerged in the Bay of Bengal in 1992 and has since spread to several countries in Asia. It is not known whether this strain has the potential to become the next pandemic strain. Countries are encouraged to test for serogroups O1 and O139 when diagnosing V. cholerae infection (2). In the United States, 1/10 cases were caused by the O139 serogroup. So far no cases caused by V. cholerae O139 have been identified in Africa (WORLD HEALTH ORGANIZATION Geneva (2009)).

Recently,5 newly evolved strains have been discovered in Bangladesh. These variant El Tor strains express the toxin formerly produced by classical strains and appear to be more virulent. Observations suggest that these strains, already identified in some parts of East Africa and Asia, cause more severe episodes of cholera with higher CFRs (WORLD HEALTH ORGANIZATION Geneva (2009)).

Multidrug-resistance has also recently emerged in Bangladesh. Strains have been isolated from patients hospitalized in Dhaka; these patients had longer stays, presented with excess purging and required more intravenous fluids for cure (WORLD HEALTH ORGANIZATION Geneva (2009)).

Cholera should be suspected when a child older than 5 years or an adult develops severe dehydration from acute watery diarrhoea (usually with vomiting), or any patient older than 2 years has acute watery diarrhoea when cholera is known to be occurring in the area. Younger children also develop cholera, but the illness may be difficult to distinguish from other causes of acute watery diarrhoea, especially rotavirus ( World Health Organization (2005)).

Cholera diarrhoea is due to the action of a toxin that acts on all animal cells by stimulating the enzyme adenylate cyclase, which catalyses the production oc cyclic AMP from ATP. In intestinal brush border cells raised cyclic AMP levels result in increased secretion of chloride ions, leading to fluid accumulation in the gut. Escherichia coli produces a similar toxin (The nature and action of cholera toxin - PubMed).

Most persons infected with the cholera bacterium have mild diarrhea or no symptoms at all. Only a small proportion, about 10%, of persons infected with Vibrio cholerae O1 may have illness requiring treatment at a health center. Cholera patients should be evaluated and treated quickly. With proper treatment, even severely ill patients can be saved (Treatment | Cholera | CDC).

Cholera differs from acute diarrhoea of other causes in three ways: • it occurs in large epidemics that involve both children and adults; • voluminous watery diarrhoea may occur, leading rapidly to severe dehydration with hypovolaemic shock; • for cases with severe dehydration appropriate antibiotics may shorten the duration of the illness ( World Health Organization (2005)).

For patients with severe dehydration and shock, the initial intravenous infusion should be given very rapidly to restore an adequate blood volume, as evidenced by normal blood pressure and a strong radial pulse. Typically, an adult weighing 50 kg and with severe dehydration would have an estimated fluid deficit of five litres. Of this, two litres should be given within 30 minutes, and the remainder within three hours ( World Health Organization (2005)).

With cholera, unusually large amounts of ORS solution may be required to replace large continuing losses of watery stool after dehydration is corrected. The amount of stool lost is greatest in the first 24 hours of illness, being largest in patients who present with severe dehydration. During this period, the average fluid requirement of such patients is 200 ml/kg of body weight, but some need 350 ml/kg or more. Patients whose ongoing stool losses are in this range, or higher, usually require intravenous maintenance therapy using Ringer's Lactate Solution with added potassium chloride. Additional potassium can also be provided by ORS solution as soon as the patient can drink ( World Health Organization (2005)).

After being rehydrated, patients should be reassessed for signs of dehydration at least every 1-2 hours, and more often if there is profuse ongoing diarrhoea. If signs of dehydration reappear, ORS solution should be given more rapidly. If patients become tired, vomit frequently or develop abdominal distension, ORS solution should be stopped and rehydration should be given IV with Ringer's Lactate Solution (50 ml/kg in three hours), with added potassium chloride. After this it is usually possible to resume treatment with ORS solution. If possible, suspected cholera patients should be treated under observation until diarrhoea stops, or is infrequent and of small volume. This is especially important for those who present with severe dehydration ( World Health Organization (2005)).

All cases of suspected cholera with severe dehydration should receive an oral antimicrobial known to be effective against strains of Vibrio cholerae in the area (Annex 7). This reduces the total volume of stool passed, causes diarrhoea to stop within 48 hours, and shortens the period of faecal excretion of V. cholerae. The first dose should be given as soon as vomiting stops, which is usually 4-6 hours after starting rehydration therapy (World Health Organization (2005)).

Tetracycline has been shown to be an effective treatment for cholera and is superior to furazolidone, cholamphenicol, and sulfaguanidine in reducing cholera morbidity. Treatment with a single 300-mg dose of doxycycline has been shown to be equivalent to tetracycline treatment and is now recommended as first-line treatment in adults, including pregnant women, and in children. Previously, antibiotics of the tetracycline class were not recommended for pregnant women due to teratogenic effects, or in children due to dental discoloration. In a recent systematic review among pregnant women and young children receiving doxycycline, there was no correlation between the use of doxycycline and teratogenic effects or dental staining in children. Erythromycin is effective for cholera treatment and is also an appropriate alternative regimen for adults, including pregnant women, and children. Orfloxacin, trimethoprim-sulfamethoxazole (TMP-SMX), and ciprofloxacin are effective, but doxycycline offers advantages related to ease of administration and comparable or superior effectiveness. Recently, azithromycin has been shown to be more effective than erythromycin and ciprofloxacin. Resistance to tetracycline and other antimicrobial agents among Vibrio cholerae has been demonstrated in both endemic and epidemic cholera settings. Resistance can result from the accumulation of selected mutations over time, or from the acquisition of genetic elements such as plasmids, introns, or conjugative elements, which confer rapid spread of resistance. A likely risk factor for antimicrobial resistance is widespread use of antibiotics, including mass distribution for prophylaxis in asymptomatic individuals (Treatment | Cholera | CDC).

When available, supplementation (20 mg zinc per day in children 6 months or older) should be started immediately (Treatment | Cholera | CDC).

A study in Bangladesh showed that zinc supplementation significantly reduced the duration and severity of diarrhea in children suffering from cholera. The study was conducted with 179 children, 3-14 years old, who were admitted to a hospital within 24 hours of the onset of cholera symptoms. In the study, all children received antibiotics and rehydration therapy as needed, but those in the intervention group also received zinc supplementation. Children who received zinc supplementation had 8 fewer hours of diarrheal illness and 10% less diarrheal stool volume, on average. Zinc has also been shown to have a similar effect in children with diarrhea caused by infections other than cholera, and is recommended for the treatment of pediatric diarrhea more generally (Treatment | Cholera | CDC).

The diagnosis of patients with severe cholera (cholera gravis) is usually not difficult because of the sudden and dramatic signs and symptoms. Patients usually come for treatment after only a few hours with diarrhea, vomiting, thirst and giddiness, and weakness. The onset is sudden and the diarrhea is generally described as painless, in contrast to shigellosis, in which defecation is associated with many cramps. The diarrhea stool is at first loose, then it becomes more watery and takes on the appearance of "rice water". Likewise the vomit has the same appearance. As the disease worsens and the dehydration becomes more severe, the patient's voice becomes weak, his level of consciousness fades, and he is too weak to hold himself upright. The breathing is noticeably labored. The patient may complain of a dry mouth, and urine flow stops (Dhiman Barua, William B. Greenough III (2013), cholera).



تشخیص بیماران با وبا شدید به دلیل علائم و نشانه های ناگهانی و مشخص آن، کار دشواری نیست. بیماران معمولا فقط پس از چند ساعت با اسهال، استفراغ،تشنگی، چرکی و ضعف برای درمان مراجعه می کنند. شروع آن عمدتا ناگهانی است و اسهال به صورت کلی بدون درد توصیف می شود، برخلاف شیگلوز که اسهال همراه با ان.اع گرفتگی ها توصیف می شود. حالت اسهال در ابتدا سست است ، سپس آبکی تر می شود و شکل "آب برنج" به خود می گیرد. به همین ترتیب استفراغ نیز همان شکل را دارد. با بدتر شدن بیماری و افزایش شدت کم آبی بدن، صدای بیمار ضعیف می شود همچنین سطح هوشیاری او کمتر می شود در این حالت بیمار، بسیار ضعیف است و نمی تواند خود را به حالت ایستاده نگه دارد. سیستم تنفس شخص به طرز محسوسی فعال می شود. بیمار ممکن است از خشکی دهان شکایت کند و جریان ادرارش متوقف شود.



On examination the cholera patient appears extremely weak, hoarse, and thirsty, often experiencing severe muscle cramps. There is evidence of dehydration, e.g.., decreased skin turgor, sunken eyes, dry mucous membranes, and a weak or undetectable radial pulse. Kussmaul respirations may be evident. The dramatic aspect of the disease is the rapidity with which a healthy person can become so sick. The appearance is sometimes described as cadaveric (Dhiman Barua, William B. Greenough III (2013), cholera).

The case described above is typical of cholera, but much less severe episodes of cholera are more common and the mild cases cannot be distinguished clinically from other acute watery diarrheal diseases. Also, a few other etiologic agents can also cause a cholera gravis syndrome, most notably the enterotoxigenic E. coli. For management of the individual patient, however, it is not necessary to know with certainty whether the acute diarrheal syndrome was due to a Vibrio cholerae or another agent. The assessment of the patient with the acute dehydrating diarrhea syndrome consists of evaluating him for signs of dehydration, estimating the severity of the dehydration, and then estimating the fluid requirements. This must be accomplished rapidly since the severely dehydrated patient may literally be within minutes of death unless hydration is instituted immediately. One estimates the degree of dehydration through a clinical assessment of the signs of dehydration. Laboratory tests (hematocrit, plasma specific gravity, or serum pro- teins) are useful in research studies for comparing groups of patients, but the clinician must formulate a treatment plan for the individual patient based on the clinical examination (Dhiman Barua, William B. Greenough III (2013), cholera).

Several oral cholera vaccines have been developed and proved to be safe, immunogenic and effective. Only 2 of these are available globally, and only 1 has been prequalified by WHO and thus is qualified for purchase by United Nations agencies. This vaccine has been used in several mass vaccination campaigns with the support of WHO, and its use has enabled the collection of new evidence on the use of oral cholera vaccines as an additional public health tool to protect populations at high risk for outbreaks (WORLD HEALTH ORGANIZATION Geneva (2009)).

In view of recent major outbreaks as well as emerging variant strains that seem more virulent, WHO recommends that immunization with cholera vaccines be used along with recommended control measures in areas where cholera is endemic as well as in areas at risk for outbreaks (WORLD HEALTH ORGANIZATION Geneva (2009)).

Cholera is globally underreported, a situation that has an adverse impact on control efforts. The open and transparent exchange of information on the occurrence of cholera is critical to improving control measures and diminishing stigma. In addition to its severe toll on public health, cholera causes serious social and economic disruption. Outbreaks may cause panic, and this may generate reactions such as quarantine, excessive isolation, and the use of mass chemoprophylaxis. These inappropriate responses can be avoided by providing adequate and timely information to policy-makers, decision-makers, the media and the public. The transparent flow of information can demystify cholera and foster a more rational approach to prevention, preparedness, early detection as well as a timely response to outbreaks (WORLD HEALTH ORGANIZATION Geneva (2009)).
